slow down on your approach and try not to lean back as your making your throw and get that left arm down by yor side when its out it slows your rotation down. good luck and keep practicing.
Two words, Balance and Follow-Through. Try rotating in a balanced manner over your plant foot (you seem to pull off to your right) finish your rotation with your left shoulder pointing at your target to complete your follow-through.

Your form is not all that bad. It is a bit jerky and unbalanced. It looks like you have good snap.
Rather than trying to throw hard, instead work on throwing smooth, flat and balanced. Be graceful, like a dancer.
